There are 3 ways to obtain your UCI Medical Records:

1. MyChart :https://my.ucihealth.org/UCI/Signup or if you have a MyChart account https://www.myucihealth.org/mychart you can also contact customer service at 1.833.469.2478 24 hours a day, 7 days a week.

2. Health Information Management Services Vital Chart secure virtual portal :(click here) or

3. You can request your records via mail or fax. To Mail or fax your written requests:
